All first-time participants with disabilities must go through an intake process prior to joining City activities to ensure our program can meet the participants needs. Simply reach out to our Inclusion Coordinator to schedule the intake at least 2 weeks in advance to the start of the activity.*

If you are a returning family and have already gone through the intake process, then when registering make sure you include a specific diagnosis or “requesting companion” for the following question:

Describe any ADA (Americans with Disabilities Act) accommodations or any physical, psychiatric, behavioral, or other concerns for which you will make a special request for a change or adaptations.

The participant and at least one parent or guardian must be present during the intake. During the intake, a parent or guardian will talk more in depth about the participant and their needs with the Inclusion Coordinator. The participant will get a chance to play and hang out with another staff member in the same room, this is done to observe how the participant behaves in a new environment with new people. At the end of the intake, all parties will discuss the best action plan for the participant. In the case that the Inclusion Program cannot meet the participant’s needs, the Inclusion Coordinator will provide the family with additional resources for other organizations and programs in the area that may be a better fit.

All intakes are conducted in person at the Activity Center in Bohrer Park and take approximately 45 minutes.
