When booking your appointment, please consider of the following:

• If you are unable to make your appointment, please call to cancel or reschedule.
• Make sure to show up for your appointment on time. If you are late, we may have to move onto the next applicant, and you will need to make another appointment.
• Make sure you have all components of your application ready before you arrive.
• All applicants, including children, minors age 0-15 and or a translator must be present to complete the application process.
• If both parents are listed on the birth certificate, both parents must appear with the minor applicant OR one parent for minors ages 16-17. If a parent is unable to attend, a notarized form DS-3053 along with a copy of the front and back of the parents ID must accompany the application. Proof of relationship includes birth certificates and adoption papers.
• Applications must be completed in BLACK ink only with no errors or mistakes at all. If there is a mistake or even one error a new application must be completed.

To apply for a passport, you will need:

1. A completed passport application. For updated forms & requirements check travel.state.gov. Note: DO NOT sign application prior to appointment, signature must be witnessed by authorized Passport Agent.

2. Evidence of United States citizenship in the form of:

• A certified original birth certificate with the seal
• A certified naturalization certificate with the seal
• A certified consular report of birth abroad and certificate of citizenship

3. One photograph:

• Color
• 2x2 inches in size
• Clear, front view, full face without head coverings or eyeglasses, white background, no smiles (selfie photos are not permitted)

4. Proof of identity containing your signature & physical description or photograph in the form of:

• A previous U.S. passport
• A Certificate of Naturalization or Citizenship
• A current driver’s license
• A current government identification card
• A current military ID (only for dependents)

5. Fees

• Passport application fee (Payable to U.S. Department of State by personal check or money order only)
  
• Passport processing fee (Payable to the City of Gaithersburg by cash, credit card, money order, or personal check)
  Note: Passport application fees and processing fees must be paid separately

• Expedited Services: $60 per application - Payable to the U.S. Department of State (This fee may be combined with the passport application payment).
• Overnight Service: The cost for overnight service from the Department of State to the applicant is $22.05 (This fee may be combined with the passport application payment).
